Appointments: a. Appointments can be scheduled online, by phone, or in person. b. Cancellations must be made at least 24 hours in advance to avoid cancellation fees. c. Late arrivals may result in rescheduling of appointments at the discretion of the healthcare provider.
Confidentiality: a. Patient medical records are confidential and will not be disclosed without the patient’s consent, except as required by law. b. Electronic communication, including email, is not guaranteed to be secure. Patients should avoid sending sensitive information through unsecured channels.
Payment and Insurance: a. Payment is due at the time of service unless other arrangements have been made. b. The clinic accepts various insurance plans; patients are responsible for understanding their coverage and co-payments. c. Uninsured patients may be eligible for discounted rates based on financial need.
Communication: a. The clinic may communicate with patients via phone, email, or text message for appointment reminders, test results, and other relevant information. b. Patients can opt-out of non-essential communication but are encouraged to receive important health-related updates.
Emergency Situations: a. In case of a medical emergency, call 911 or go to the nearest emergency room. b. The clinic should be notified of any emergency room visits or hospitalizations as soon as possible.
Termination of Services: a. The clinic reserves the right to terminate services if a patient engages in disruptive behavior, fails to comply with treatment plans, or violates clinic policies.
Changes to Terms and Conditions: a. The clinic reserves the right to modify these Terms and Conditions at any time. Patients will be notified of any changes.
Governing Law: a. These Terms and Conditions are governed by the laws of Washington. Any disputes shall be resolved in the courts of Washington.
